Time SUMMARY:
Under the direction of the Program Director/COO, this position is responsible for providing enhanced care and coordination to unsheltered adults diagnosed with substance use and/or a mental health disorder. The Care Coordinator is responsible for screening/assessing client housing needs with the VI-SPIDAT, ensuring clients eligible for housing are in the HMIS system and providing transition services as clients progress through the Aloha House continuum of care. This position will collaborate with community resources in order to help unsheltered clients get their needs met.
ESSENTIAL DUTIES AND RESPONSIBILITIES
1. Assists in screening/assessing the housing status of adults with substance use and/or mental health disorders. 2. Verify client data in HMIS; if not present and eligible for housing, intake client. 3. Create housing goals with clients and document in WITS Health & Wellness plan. 4. Provide ongoing face to face meetings with the client on a regular basis to assess the progress made in reaching service goals so that the service plan can be modified as necessary to ensure that the goals and objectives are being achieved. 5. Provide care coordination to unsheltered clients on a weekly/monthly basis. 6. Documents all services provided in WITS within 24 hours of contact, including face-to-face, collateral and networking contacts. Demonstrate appropriate & complete documentation- related information including service plans and reviews, progress notes, correspondence, releases of information, etc.… in compliance with agency standards. 7. Assist with transition and provide ongoing care as clients progress through Aloha House COC. 8. Perform follow ups with clients after they leave
AHI COC. 9.
Maintain a spreadsheet of all billable services rendered. 10. Maintain communication and collaboration with community service providers regarding client referrals and provision of services. 11. Maintain collaborative relationships and communication with community referring agencies to formulate appropriate collaborative intervention strategies. 12. Attend all required meetings, trainings and supervision. 13. Adheres to Code of Ethics in Employee Handbook.
CERTIFICATE/LICENSE/REGISTRATION
1. CPR/FA Certification 2. Current driver's license, clean driving abstract and current car insurance.

EDUCATION AND/OR EXPERIENCE
1. Master's Degree from an accredited university or college, in social work, counseling, psychology, or related behavioral sciences field. 2. One and half years of experience working with clients in human service setting.
