Job Title :
Caregiver Specialist, Southern Kane and Kendall Counties Function :
The Caregiver Specialist maintains a caseload. The Caregiver Specialist works closely with families, caregivers, and SSA participants to determine the need and available resources. Additionally, the Caregiver Specialist provides education, outreach, and training to individuals and the community.
Reports To :
Director of Care Coordination Unit Major Duties and Responsibilities :
Position is a direct service position. Provides pertinent information about community resources to family caregivers over the telephone, in person, in-home, and/or in community settings. Screens to determine unpaid family caregiver need, eligibility, and presumptive eligibility for services. Completes evaluation of caregiver using Tailored Care (TCares) Assessment Tool, which assess caregiver burden and determine appropriate strategies and services based on caregiver responses. Through in-home, office or phone consultation using person-centered counseling, assists family caregivers to develop an appropriate plan to meet their needs. Works with other provider agencies to coordinate client services and resolve client challenges. Documenting in AgingIS and completing Daily Service Logs (DSLs)
Key Skills Required :
Bachelor's Degree required (Master's Degree preferred) Must be organized, mature and professional Will represent the agency in the community by following Senior Services Mission Statement and FISH philosophy. Excellent oral and written communications skills, customer service skills Able to handle multiple issues simultaneously Detail oriented - maintain thorough, accurate records Good judgment and decision-making, must be able to prioritize important calls, and tasks. Level-headed, able to act/interact appropriately in emotional situations. Able to deliver uncomfortable information in such a way as to minimize adverse reactions. Responsible for managing caregiver funding resources by utilizing spreadsheets. Responsible for submitting funding requests to Accounts Payable (AP) office
Schedule :
Monday to Friday 8:00 am - 4:00 pm; 7.50-hour workdays Full-time position only Volunteering for agency events is desired
