In this vital role, you will coordinate and deliver comprehensive care management services to patients with complex, chronic health conditions. Your expertise will ensure seamless communication among healthcare providers, patients, and caregivers, ultimately improving health outcomes and enhancing patient satisfaction. The ideal candidate is energetic, detail-oriented, and possesses a strong foundation in clinical care, medical coding, and healthcare systems. This position offers an exciting opportunity to make a meaningful difference in patient lives through proactive care coordination and management. Duties Conduct thorough patient assessments to identify needs related to chronic conditions and develop personalized care plans. Collaborate with healthcare providers across hospital departments, including ICU, emergency medicine, pediatrics, and behavioral health, to coordinate ongoing patient care. Manage patient intake processes, discharge planning, and follow-up activities utilizing Electronic Health Record (EHR) systems while ensuring compliance with HIPAA regulations. Utilize CPT coding, ICD-9/10 coding, and ICD coding expertise to accurately document diagnoses and procedures for billing and utilization review purposes. Facilitate case management activities such as utilization management, review of hospital stays (including Level I and Level II trauma centers), and managed care coordination. Support patients through discharge planning, social work referrals, hospice care coordination, and behavioral health services to promote continuity of care. Maintain detailed documentation of patient interactions, interventions, and outcomes to support quality improvement initiatives. Skills Extensive experience in critical care environments such as ICU, emergency medicine, or trauma centers with a focus on hospital medicine or acute care settings. Strong nursing background with knowledge of medical terminology, discharge planning, intake procedures, and case management protocols. Proficiency in EMR/EHR systems along with familiarity with HIPAA compliance standards for patient privacy. In-depth understanding of
ICD-9/10
coding standards and CPT coding practices for accurate documentation and billing. Experience working with social work teams, behavioral health providers, managed care organizations, and utilization review processes. Ability to navigate complex healthcare environments including Level I/II trauma centers and pediatric units such as PICU or OR experience is highly desirable. Excellent communication skills to effectively collaborate across multidisciplinary teams in fast-paced hospital settings. Join us in transforming patient care through proactive management!
