The League for the Blind & Disabled Independent Living - Senior Blind Services Rehabilitation Teacher The Independent Living division of The League is seeking a dedicated, hard-working and self-motivated Rehabilitation Teacherto assist with independent living activities, individual advocacy, peer counseling and case coordination services for older individuals who are blind or low vision who live in North Central Indiana (Elkhart, Marshall, Kosciusko, St. Joe and LaPorte counties). The ideal candidate should be motivated to serve our community. Prefer a qualified person with a disability. The League is Indiana's largest Center for Independent Living headquartered in Fort Wayne since 1950. This position expands the organization's mission outside of the core 11 counties in Northeast Indiana, allowing us to provide independent living services in underserved rural communities. Essential Duties/Skills/Responsibilities/Qualifications Experience working with seniors and/or individuals with vision loss. Conducts initial assessment to determine consumer's individual needs, assisting consumer in developing initial, intermediate and long-range goals, tracking and monitoring progress. Conducts low vision evaluations, establishing needs for daily living and adaptive aids, devises & equipment training. Organizes, leads and maintains local support groups for consumers and family members managing senior vision loss. Offers information and referral (I&R) services; participates in individual and systems advocacy; and provides individual peer support. Represents the League at community health fairs, conducting public education, awareness and outreach activities. Provides community organizations with training related to vision loss issues in older adults. Maintains accurate, up to date consumer files, community activities and I&R contacts according to procedures . Bachelor's degree in Rehabilitation Teaching or related fields or an equivalent combination of education and experience. Prefer a qualified person with a disability and/or an individual 55 years old or older (preference as identified by state funding source).
