To Be Determined Job Description The Ashburnham-Westminster Regional School District is seeking an 18-22 Program Transition Specialist to manage services for young adults with disabilities, focusing on fostering independence in employment, education, and community living. The role conducts assessments, develops IEP transition goals, supervises staff, and connects families with adult service agencies to ensure a seamless transition from school to post-secondary life. Key Responsibilities
Individualized Transition Planning:
Develop, implement, and monitor transition IEP goals focused on vocational skills, independent living, and post-secondary education.
Vocational & Community Training:
Oversee staff and facilitate work-based learning, including job shadowing, internships, and community-based instruction to build work readiness, as well as paid employment opportunities.
Agency Linkage & Advocacy:
Act as a liaison between families, community partners, and state agencies (e.g., developmental services, vocational rehabilitation) to connect students with adult resources.
Transition Assessments:
Conduct formal and informal age-appropriate transition assessments to identify student interests, strengths, and needs to inform programming.
Skill Development:
Instruct students in self-advocacy, independent living skills, transportation training, and social skills.
Curriculum Development:
Create functional life skills and community-based curriculum tailored to students aged 18-22.
Case Management & Compliance:
Oversee student caseloads, maintain necessary documentation, and ensure adherence to state regulations (e.g., Chapter 688). Maintain documentation and monitor student progress toward post-secondary goals.
