Care Enrollment Specialist Hyannis, MA Apply The Access to Health Insurance & Care Enrollment Specialist\'s main responsibilities are: screen uninsured & underinsured individuals for Public Benefit Health & Dental Insurance Programs; assist customers to apply for & enroll in the program they are eligible for & assist them to maintain their health insurance; educate the enrollee in the health care system & link them to a Primary Care Physician; & participate in community outreach & enrollment events. In addition, conduct a comprehensive intake & provide resource information & referral to local agencies for any additional supports needed. Comply with all mandates required by funders to include certifications, outreach, enrollment, & consumer education. Position requires flexibility to work weekends, evenings, & travel as necessary to fulfill contract obligations.

MAJOR DUTIES

Conduct a comprehensive intake with potential applicants, screening for health & dental insurance & other services available through CACCI or other community resources. Become a certified MA Health Connector Navigator in order to enroll clients into appropriate public health insurance programs & assist consumers with all aspects of maintaining their health insurance; once gained, maintain Navigator certification. Assist customers in applying & enrolling into appropriate public health insurance programs; educate them on the health care system & health insurance options promoting health insurance literacy; & link them to a primary care provider. Educate public health insurance consumers on the Massachusetts health care system & health insurance options promoting health insurance literacy; &, when necessary, link them to a primary care provider. Participate in community outreach & enrollment events. Out post at sites as required. Understand, implement, & adhere to program policies, procedures, & regulations including HIPPA Policies. Maintain data base files & keeps records in order to facilitate reporting requirements. Participate in trainings, meetings, & forums addressing health access & public health insurance as appropriate. As appropriate, respond to various issues that present themselves as CACCI serves the immigrant population of Cape Cod & Nantucket to include cultural competency & translation assistance. As a member of the Client Self Sufficiency Department (CSS), the Access to Health Insurance & Care Enrollment Specialist assists the department by referring customers to staff who provide SNAP enrollment, VITA free income tax prep, Holiday assistance, & other programs to support client self-sufficiency. At times this position may be asked to provide enrollment assistance for Holiday & other special projects. Assist with Reception coverage as needed. Adhere to Agency Policy & Procedures. Act as a role model within & outside the Organization. Perform duties as workload necessitates. Maintain a positive & respectful attitude. Communicate regularly with supervisor about Department issues. Demonstrate flexible & efficient time management & ability to prioritize workload. Consistently report to work on time prepared to perform duties of position. Meets Department productivity standards.

KNOWLEDGE, SKILLS & ABILITIES

Excellent interpersonal & communication skills. Demonstrable organizational skills for multi-tasked position. Ability to learn varied complex systems quickly; ability to be trained to pass the required Health Connector Authority Navigator certification. Ability to work independently; Self-motivated. Performs a variety of tasks with a minimal amount of supervision. Knowledge of computer operations, word & data base systems. Valid US driver\'s license & reliable transportation.

SUGGESTED TRAINING & EXPERIENCE A
