Transition Specialist Job Description:
The role of the Transition Specialist is to improve/enhance the transition experience to post school life for students with disabilities.
Performance Responsibilities:
  • To provide staff training to a variety of audiences and in a variety of formats regarding transition planning.
  • To assist staff and communicate with students and parents regarding the program design, implementation, and monitoring of special education programs for students with disabilities transitioning to post school life.
  • To develop and coordinate community partnerships to promote transition opportunities for students.
  • Through a variety of methods (individual consultation, seminars, topic related trainings, etc.), to increase the knowledge base of all staff working in the public schools regarding the unique needs of students with disabilities transitioning to post school life.
  • To develop community-based training and work-based learning opportunities (i.e. internships) based on student's identified interests and needs.
  • To coordinate and collaborate with agencies and community partners in identifying community-based and/or work-based opportunities and resources for students.
  • To observe target students in a variety of settings for purposes of both data collection and to provide consultation to educational team members, regarding curriculum and instruction on transitional needs.
  • To conduct transition planning assessments and with input from team, support the development of appropriate transition plans.
  • To participate in select Team meetings for the purpose of assessing student progress and developing or revising IEPs.
  • To provide ongoing data/ information to district administrators regarding ongoing and/or changing student needs for the purpose of school year and summer program planning.
  • In conjunction with the child's Team, to facilitate the transition planning for students, from high school to post school life, as well as grade to grade, building to building.
  • To assess the need for and then develop/ lead a family group (as needs determine) for the purpose of sharing information and support, and/or to provide training for families on post school life options.
  • To design and monitor a variety of assessment systems to document student progress.
  • Any/ all other duties assigned by Special Education Administrator related to the educational planning of students ages 14-21.
Qualifications:
  • Bachelor's Degree or higher in Special Education or Related Field
  • License as a Special Education Teacher preferred and/or current Transition Specialist endorsement or candidate strongly preferred
  • Minimum 5 years' experience with students aged 14-21
  • Strong collaboration/consultation skills
  • Excellent organizational skills
  • Excellent communication and presentation skills
  • both oral and written
  • Extensive knowledge of Special education regulations/ LRE/ FAPE
  • Spanish or Portuguese speaking preferred The Superintendent of Schools reserves the right to waive a position qualification or determine that there is an acceptable alternative to that qualification.
If the Superintendent intends to waive a qualification, such intent will be shared with the Fall River School Committee.
