Home Care Nurse - Home Visits Specialist Sewa Home Care Lowell, MA Job Details Part-time | Per diem $50 - $54 an hour 8 hours ago Qualifications Electronic health records (EHR) management RN License Care documentation Diagnostic evaluation Medical scheduling Records management Health record tracking LPN Managing patient records Full Job Description Job Overview We are seeking a dedicated and compassionate Home Care Nurse to provide weekly in-home visits for patients requiring ongoing health monitoring and support. This role is perfect for a nurse passionate about community-based care and committed to enhancing patients' quality of life in the comfort of their own homes.
Duties :
Medication Management:
Assist patients in organizing and safely administering medications. Review and confirm adherence to prescribed medication regimens. Educate patients and caregivers about proper medication use and potential side effects.
Health Monitoring:
Check vital signs, including blood pressure, temperature, and heart rate. Monitor diabetic readings such as blood sugar levels and provide guidance for optimal control. Perform regular skin assessments to identify issues like pressure ulcers or signs of infection.
Patient Assessment:
Conduct comprehensive assessments during each visit to evaluate the patient's overall health status. Identify any changes or emerging health concerns that require further attention.
Documentation & Reporting:
Maintain accurate and up-to-date patient records detailing each visit, observations, and care provided. Communicate findings to the broader healthcare team to ensure continuity of care.
Patient Education & Support:
Provide tailored education on managing chronic conditions, such as diabetes. Offer advice on nutrition, exercise, and lifestyle modifications to promote overall wellness. Address any patient or family questions, offering reassurance and support.
Coordination of Care :
Collaborate with physicians, pharmacists, and other healthcare providers to update care plans. Assist in scheduling follow-up appointments, lab tests, or referrals as needed.
Emergency Preparedness:
Recognize and respond to potential emergencies, providing immediate care or referrals when necessary. Ensure patients and caregivers know the appropriate steps to take in urgent situations.
Qualifications:
Current and valid nursing license (RN/LPN). Proven experience in home healthcare, community nursing, or similar roles. Strong clinical assessment skills and ability to identify changes in patient condition. Excellent communication and interpersonal skills. Ability to work independently and manage time effectively.
