Job Summary:
The Diabetes Care and Education Specialist (DCES) is a healthcare professional with advanced knowledge and credentialing to support individuals across the diabetes spectrum. This role focuses on creating personalized self-management plans, coordinating care, interpreting patient data, conducting educational assessments, and promoting successful self-management. The DCES works within a multidisciplinary team and delivers care in accordance with professional standards and hospital values.
Key Responsibilities:
Assess and educate patients in DSMES/T (RD/RN) and MNT (RD only), tailoring education to age, culture, and developmental stage. Provide individualized education using interactive, collaborative, skill-based methods. Complete clinical and CQI documentation promptly; adjust follow-up plans based on patient progress. Communicate patient progress to referring physicians after each visit (or after class #4 for Medicare participants). Collaborate with colleagues on treatment plans, events, goal-setting, and CQI initiatives within the Diabetes Program and Tevis Center for Wellness. Participate in community education and screening events as needed. Attend multidisciplinary conferences and represent the hospital professionally in internal and external meetings. Maintain professional development by retaining CDCES certification or completing at least 15 approved contact hours annually. Perform other duties as assigned to support program operations and patient care excellence.
Qualifications:
Education:
Bachelor's degree in nutrition or a related field
Licenses & Certifications:
Licensed Dietitian-Nutritionist (Maryland license or eligibility) Certified Diabetes Care and Education Specialist (CDCES) Basic Life Support (BLS)
Registered Dietitian Skills & Competencies:
Strong clinical knowledge in diabetes education and nutrition therapy Excellent communication, collaboration, and patient engagement skills Ability to work independently and within a multidisciplinary team Commitment to continuous professional development and quality improvement
