SUMMARY:
The Help Me Grow (HMG) Care Coordinator supports the national Help Me Grow model which utilizes and builds on existing resources to develop and enhance a comprehensive approach to early childhood system-building, The Care Coordinator will connect families and young children with the services they are eligible to receive. This role will implement and expand the Help Me Grow system in Washtenaw County by answering questions from families, providing follow up services related to developmental screenings and referrals, tracking data, and providing cross-sector coordination to ensure families are seamlessly connected to the full scope of services within the intermediate district region.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Demonstrates a strong commitment to equity, social justice, and inclusion in all practices and position responsibilities. Demonstrates the ability to examine how his/her/their position (directly or indirectly) impacts educational inequities in student achievement outcomes. Engages in reflection and ongoing learning and development on critical concepts and terms identified in the Washtenaw ISD Educational Equity Policy including concepts such as cultural proficiency, racial equity, and systemic and structural inequities. Possesses an extensive knowledge and understanding of Early On, Head Start PN-3, the Great Start Readiness Program (GSRP) and the systems that support them. Coordinates with the WISD Family and Community Partnerships Specialist. Responds to the needs of families being served by WISD's PN-3 programs. Develops, strengthens, and maintains connections with community partners. Engages stakeholders, establishes relationships with community partners, and identifies resources that can be made available to families. Initiates and tracks community partnership agreements when appropriate. Coordinates with the Health Services Specialist to deepen and maintain connections to local hospitals, pediatricians, and dental offices. Coordinates with the home-based service providers (Early Intervention and Early Head Start) to expand on existing relationships with local agencies, schools, community organizations, and programs. Partners with the Community and School Partnerships department to explore connections and collaborate on joint projects. Maintains the Help Me Grow Washtenaw Ages and Stages (ASQ) online database. Processes and responds to Ages and Stages developmental and behavioral Questionnaires submitted by community members. Supports WISD Head Start, Early Head Start, and GSRP programs in documenting developmental screening results. Supports Child Find activities to identify infants, toddlers, and preschool aged children with potential developmental delays. Attends community networks and other community organization meetings for the purpose of sharing information and expanding the HMG network. Submits verbal and/or written reports on monitoring and follow-up to supervisor and/or program Assistant Director on HMG activities. Maintains and analyzes data related to referrals, evaluations and assessments, and service coordination. Performs duties with awareness of all district and Board of Education requirements. Maintains regular predictable attendance.

EXPERIENCE
Bachelor's degree in early childhood, social work, or related field is preferred, but not required. Possesses a minimum of one (1) year or more of relevant experience in early childhood, social work, or a related field is required. Possesses extensive knowledge of community resources in Washtenaw County demonstrated by a minimum of one (1) year of experience supporting families in Washtenaw County. Possesses experience in direct service delivery to families of young children. Possesses knowledge and experience with program data management and analysis.
LANGUAGE SKILLS
Possesses bilingual ability (in Arabic and/or Spanish) is preferred. Demonstrates the ability to communicate ideas and instructions orally and in writing. Demonstrates the ability to draft reports and business correspondence. Demonstrates the ability to speak before groups of individuals.
TECHNICAL SKILLS
Demonstrates the ability to accurately use district-wide electronic reporting systems (Child Plus, MICR, Power-school). Demonstrates the ability to work with computer programs as they relate to program responsibilities. Demonstrates the ability to use a personal (PC) in a networked environment to utilize the Internet and other electronic communications mechanisms, such as email, conferencing, and/or messaging boards. Demonstrates the ability to use computer technology for research, data management, communications, and other instruction.
MATHEMATICAL SKILLS
: Demonstrates the ability to work within the contractual budget. Demonstrates the ability calculate figures and amounts such as percentages. Demonstrates the ability produce and interpret graphs, charts, spreadsheets, and other mathematical reports..
REASONING ABILITY
Demonstrates the ability to follow data collection information to support functional behavioral assessment. Demonstrates ability to define problems, collect data, establish facts, and draw valid conclusions. Demonstrates ability to interpret an extensive variety of instructions. Demonstrates the ability to read, analyze, and interpret general social services related journals and technical procedures. Demonstrates the ability to summarize conclusions and instructions, then make recommendations and develop implementation strategies. Demonstrates the ability to perform duties with awareness of all district and Board of Education requirements.
INTERPERSONAL SKILLS
Demonstrates the ability to establish and maintain effective working relationships with children, families, and the community. communicate clearly and concisely both in oral and written form. Demonstrates ability and desire to work in a collaborative team with others. Demonstrates excellent customer service and communication skills.
PHYSICAL DEMANDS

Travel by car, bus, airplane, or train may also be required associated with attendance at conferences, meetings, and other duties conducted at distant locations in and out of state and in some cases where some overnight travel may be required. TERMS:
Salary and other employment conditions to be established by the Board of Education as reflected in the Non-Affiliated and Early Childhood Manual . Starting Salary ranging from (dependent upon experience) $50,322 - $61,857. Washtenaw Intermediate School District is a drug-free workplace.
