Key Responsibilities:

Deliver skilled Physical Therapy interventions in the patient's home, focusing on mobility, safety, and quality of life Conduct Start of Care (SOC) assessments, admissions, and ongoing clinical evaluations Design and implement functional, client-specific treatment plans Collaborate with PTAs, nurses, and other interdisciplinary providers to adjust plans of care and ensure clinical alignment Document progress using evidence-based practices and ensure regulatory compliance, including OASIS when applicable

Preferred Qualifications:

Active Minnesota Physical Therapist license (PT or DPT) Prior experience in home health or outpatient mobile care (preferred) Demonstrated proficiency with OASIS documentation or willingness to be trained Excellent time management, documentation accuracy, and decision-making skills Valid driver's license and reliable transportation (this role involves frequent travel within a 50+ mile regional radius) This position is ideal for clinicians who value autonomy, professional respect, and the opportunity to treat patients holistically in their most meaningful setting—their home.
