- As an
- SNBC Care Coordinator,
- you will make outbound calls and respond to a telephonic member queue and crisis calls. This position combines working remotely at home (~85%) with in-person meetings with members (~15%) and occasional on-site meetings at the corporate headquarters. Residency in or near the Twin Cities, MN metro is required.
- The schedule is Monday through Friday, 8:00am - 5:00pm CST.

Primary Responsibilities:
- + Provide telephonic care coordination to MN Medicaid members with a mental health disability + Complete annual face-to-face meeting for qualified members + Manage gaps in care for members in both their mental health and medical needs (i.
Required Qualifications:
- + Master's or Doctoral Degree in Psychology, Social Work, Counseling, or related field + Active, unrestricted, independent license in
Minnesota:
LP, LPCC, LICSW, LMFT
+ 2+ years of clinical experience in a behavioral health setting + Proven ability to work in multiple computer and web-based applications simultaneously + Valid driver's license and access to reliable transportation that will allow you to travel within the Metro for annual member visits (up to 15%) and occasional in-office meetings + Live in or near the Twin Cities, MN + Dedicated, distraction-free workspace and high-speed internet at homePreferred Qualifications:
- + Case Management experience + Experience managing a caseload + Experience working with Adults with Severe and Persistent Mental Illness (SPMI) + Experience with Substance Use Disorders (SUD) + Experience working on a phone queue or crisis line + Experience working in a strength-based recovery model + Experience using motivational interviewing + Experience making home visits + Proven organizational skills + Proven ability to work independently Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc.
