80 Req #: 12106 Diabetes Care Specialist Somers Point, NJ | Shore Physicians Group Position Summary The Diabetes Care Specialist provides education, training, and ongoing support to patients initiating insulin therapy, including correction techniques, as well as training on Continuous Glucose Monitors (CGMs) and insulin pumps. This role requires a strong background in diabetes management, patient education, and clinical care to empower patients to effectively manage their condition. The Diabetes Care Specialist is responsible for delivering comprehensive care management services for patients with complex and chronic conditions. This includes developing, implementing, coordinating, monitoring, and evaluating individualized care plans to optimize health outcomes and ensure appropriate access to services across the continuum of care. Key Responsibilities
  • Educate patients and their families on diabetes management, insulin therapy, and glucose monitoring techniques
  • Conduct training sessions on CGM devices, including sensor insertion, calibration, and data interpretation
  • Analyze CGM reports to identify patterns, trends, and opportunities for improved diabetes control
  • Provide comprehensive training on insulin pump therapy, including pump operation, infusion set changes, and troubleshooting
  • Review and interpret insulin pump data downloads to optimize insulin delivery and glycemic outcomes
  • Collaborate with physicians, nurse practitioners, dietitians, and other healthcare professionals to coordinate comprehensive diabetes care
  • Provide targeted, proactive, relationship-based (longitudinal) care management for high-risk diabetic patients using established risk stratification processes
  • Support patients in successfully following individualized care plans and encourage use of self-management tools
  • Connect patients and families with community resources, medication assistance programs, and other support services
  • Partner with diabetes education programs to deliver content and track patient use of diabetes-related technologies
  • Collaborate with office leadership to improve workflow efficiency and overall care delivery
  • Assist in coordinating transitions of care for hospitalized patients with diabetes
  • Support patient scheduling processes to ensure timely access to care Skills, Experience & Educational Requirements
  • Active State Registered Nurse (RN) license required
  • Certified Diabetes Care and Education Specialist (CDCES/CDE) preferred, but not required
  • Minimum of 1 year of clinical experience in diabetes management, endocrinology, or a related specialty
  • Experience with CGM systems, insulin pumps, and diabetes management software preferred
  • Strong communication, patient education, and interpersonal skills
  • Ability to work independently, prioritize tasks, and adapt to changing patient needs
  • Commitment to continuous learning and evidence-based diabetes care
  • Proficiency in Microsoft Office (Word, Outlook, Excel) and ability to learn new software systems
  • Experience with electronic health records (EHR); Athena EMR experience preferred
  • Strong organizational, analytical, and problem-solving skills
  • Ability to thrive in a fast-paced, service-oriented environment
  • Ability to maintain effective working relationships with providers, staff, patients, and the broader medical community Salary Range $26.
