As part of the RN Wound Care/Treatment Team, the Wound Care Nurse participates in assessing and evaluating the clinical needs of residents with wounds while working with them and the rest of the care team in planning and implementing care plan that addresses the needs.
Schedule:
Full-time, 8 or 12hr shift, from
Monday-Friday Responsabilities:
Conduct comprehensive wound consultations including detailed wound assessments and recommendations for interventions and care planning. Conduct wound assessments for residents and complete all associated documentation, tracking and care planning activities. Ongoing monitoring of all assigned residents with wounds. Assess residents/caregivers knowledge of wound prevention and treatment. Provide residents/caregivers with wound prevention and treatment education as needed.
Requirements:
Must have at least 2 years' experience in wound care at a long-term care facility, SNF preferred. RN License in NY. Wound Certification preferred. Why Join Our Team?
