Job Description

Job Summary Cedar Oaks Wellness Center is seeking a highly motivated Recovery Engagement & Continuum of Care Specialist to support patient engagement, retention, and successful progression through the treatment continuum. This position plays a vital role in helping patients remain engaged in treatment, overcome barriers to recovery, and transition to the most appropriate level of care. The ideal candidate is someone who understands the recovery process, has experience as a Peer Support Specialist or in a recovery-oriented role, and can effectively build trust with patients and families. Candidates should be comfortable discussing treatment recommendations, helping patients understand the benefits of continuing care, and encouraging participation in the next appropriate level of treatment. Lived experience in recovery is strongly preferred. Primary Responsibilities Build and maintain meaningful, recovery-oriented relationships with patients to foster trust, engagement, and commitment to treatment. Support patient retention by identifying barriers to treatment participation and implementing engagement strategies. Proactively intervene with patients considering leaving treatment AMA (Against Medical Advice) or ACA (Against Clinical Advice). Educate patients and families on the benefits of continued treatment and appropriate levels of care. Assist patients in understanding and embracing recommendations for residential, PHP, IOP, outpatient, and recovery support services. Coordinate transitions between levels of care in collaboration with clinical, medical, and admissions teams. Lead and support comprehensive discharge planning to ensure continuity of care. Engage family members through education, support, and treatment planning discussions. Encourage attendance and participation in clinical programming and recovery activities. Lead or co-facilitate life skills groups, recovery groups, and community meetings. Assist patients in identifying and achieving individualized recovery goals. Provide ongoing encouragement, accountability, emotional support, and recovery coaching throughout the treatment process. Serve as a positive role model for recovery and personal growth. Qualifications Experience working in behavioral health, substance use disorder treatment, peer support, case management, admissions, or patient engagement. Ohio Certified Peer Recovery Supporter or QBHS (or ability to obtain certification) preferred. Personal recovery experience and familiarity with the recovery community strongly preferred. Knowledge of the behavioral health and substance use disorder treatment continuum of care. Understanding of residential treatment, PHP, IOP, outpatient services, and recovery support services. Ability to communicate the value and importance of continued treatment and step-down care. Excellent interpersonal, motivational, and relationship-building skills. Strong crisis intervention and problem-solving abilities. Ability to work effectively with patients, families, and multidisciplinary treatment teams.
