Job Description

QUALIFICATIONS
Required :
Possession of valid Oregon driver's license; excellent driving record and ability to be insured by the hospital's auto insurance policy; Ability to work independently without constant supervision; Ability to effectively communicate with patients, medical personnel, community partners and others they may come into contact with, in a pleasant and constructive manner; Ability to prioritize workload in a timely manner; Ability to take direction from lead, manager or director regarding work activities and procedures.
Preferred :
Health/social service experience; experience working in multi-cultural settings, bilingual.
TYPICAL PHYSICAL/MENTAL DEMANDS
Ability to drive safely throughout the geographic area defined by Sky Lakes Medical Center, in all types of weather. Must be able to climb a minimum of two flights of stairs. Must be able to work with periods of indirect supervision in patient's place of residence. Must be able to withstand constant walking on even and uneven terrain, standing, bending and lifting at least 25 lbs. regularly and 50 lbs. occasionally, push/pull DME equipment, and occasional sitting. Must be able to function during periods of stress, and exposure to potential elements in the field and have the requisite visional, auditory and fine motor skills.
ESSENTIAL JOB FUNCTIONS
Responsible for establishing trusting relationships with patients and their families and various community partner organizations. Utilizes motivational interviewing techniques to provide individual support and coaching for patients and their families. Coordinates and manages closed-loop referrals to other community organizations. Leads, teaches, and facilitates various health classes at the Wellness Center and other community partner sites. Provides transportation to necessary health and social services to qualified beneficiaries. Provides current and appropriate general health and wellness education throughout interaction with patients. Identifies barriers and implements strategies to overcome barriers in efforts to connect patients to health and social services. Works cooperatively with other clinical personnel assigned to same patient. Operates Sky Lakes owned motor vehicles in a safe and professional manner. Must be able to remain calm under stressful situations. Frequent sitting, standing, and/or driving throughout the shift required. Must be able to perform varied activities.
