Job Description

We are a dedicated community services organization committed to breaking the cycle of homelessness. Operating across three counties, we provide a compassionate bridge from life on the streets to the stability of a permanent home. Our mission is to meet individuals exactly where they are, offering a path forward through intensive support, resource navigation, and unwavering advocacy. We are looking to find an Aftercare HTNS Specialist. Aftercare Housing Transition Navigation Services (HTNS)

Full-Time, Non-Exempt Position Summary The Aftercare HTNS Specialist is a vital bridge for members transitioning from transitionalhousing into permanent, stable environments. Operating under the CalAIM (CaliforniaAdvancing and Innovating Medi-Cal) framework, you will provide intensive post-placementsupport to ensure housing retention, prevent recidivism into homelessness, and help members navigate the complexities of independent living. This role is centered on proactive communication, clinical tracking, and resource coordination to ensure our members don't just find a home—they keep it. Key Responsibilities Post-Transition Navigation & Support Conduct regular home visits and check-ins (in-person and virtual) to assess housing stability. Act as the primary point of contact for members for 6-12 months post-transition. Help members understand lease agreements, utility payments, and tenantrights/responsibilities. Mediate potential conflicts between members and landlords to prevent evictions. Case Tracking & Documentation Maintain meticulous records in accordance with DHCS (Department of Health CareServices) and CalAIM standards. Track member progress using internal databases and Required Reporting Templates(RRTs). Monitor "Early Warning Signs" (e.g., missed appointments, behavioral shifts) that could jeopardize housing. Care Coordination Collaborate with ECM Lead Care Managers to ensure the member's physical and mental health needs are met. Reconnect members to community supports such as food pantries, transportation assistance, and employment services. Assist members in developing a "Housing Sustainability Plan" tailored to their specific income and challenges. Communication & Advocacy Maintain a "warm hand-off" communication loop with the Transitional Housing team. Provide crisis intervention and rapid-response support if a member faces a housingemergency.
Required Qualifications Experience:
Minimum 2 years of experience in social services, housing navigation, or case management.
Knowledge:
Familiarity with CalAIM Community Supports, Housing First principles, and Trauma-Informed Care.
Communication:
Exceptional interpersonal skills; ability to build rapport with vulnerable populations and professional relationships with landlords.
Technical:
Proficiency in EHR (Electronic Health Record) systems and data tracking tools.
Licensing:
Valid Driver's License and reliable transportation (frequent travel to member homes required). Preferred Skills Lived experience with housing instability or recovery is highly valued. Bilingual capabilities (Spanish/English preferred). Knowledge of local Fair Housing laws and Section 8/HUD regulations. Physical Requirements & Work Environment This position involves field work. Standard office environment for administrative tasks and team meetings. •This is an in person position, remote/virtual work is not an option•
Pay:
$24.00 - $26.00 per hour
