We are seeking a highly motivated, professional, and adaptable individual to join our growing team. This is not an entry-level role. We are looking for someone who understands the healthcare industry, can communicate confidently with families and healthcare professionals, and thrives in a fast-paced environment where every day is different. What We're Looking For The ideal candidate has experience working with: Elderly populations Case management Skilled nursing discharge planning Assisted living and community placement Healthcare coordination and family support You must be: Professional, dependable, and responsible Able to pivot between multiple roles and responsibilities Comfortable communicating with hospitals, skilled nursing facilities, families, and care providers Skilled in healthcare terminology and documentation Strong in relationship-building and marketing/outreach Flexible with hours, including weekends when needed Self-motivated and able to work independently Requirements Valid driver's license with clean driving record Ability and willingness to commute throughout: Bay Area Central Valley Sacramento region Reliable, organized, and detail-oriented Strong interpersonal and communication skills Experience in healthcare placement, senior care, discharge planning, or related field preferred Bilingual skills are highly valued and considered a strong plus.
