Job Description

The Network Performance Specialist - HRSN Screening / Navigation / Care Management will work within the Social Care Network (SCN) Team in the Southern Tier region of New York and will be responsible for managing successful participation and performance of HRSN Screening / Navigation / Care Management services by participating organizations. As a local subject matter expert in this field, this role will integrate continuous improvement in program design, execution and monitoring by helping to identify best practices, promote workflow enhancements, and host stakeholder communications with contracted organizations. Screening & Referral Navigation Manager Essential Functions:
Serves as a regional expert of the 1115 waiver health-related social need (HRSN) screening, navigation, and care management services, working to ensure eligible services are delivered in a timely manner across the Southern Tier region, and provides recommendations that improve enhanced service delivery across a network of community-based organizations (CBOs). Maintains enhanced knowledge of the Social Care Navigator roles and responsibilities. Develops and deploys materials for hiring, onboarding, and training to support Social Care Navigators across the Southern Tier region and ensures that navigators are aware of programmatic requirements and best practices to ensure they meet and can maintain the defined readiness criteria. Seamlessly utilizes the Care Compass Unite Us system in support of social care navigation service delivery (screening, navigation, enhanced care management, and referrals), providing recommendations for system enhancements and consistently integrating continuous improvement. Maintains a strong understanding of NYS DOH screening, navigation and care management requirements. As needed, supports the development of communications to ensure network participants are aware of requirements and leading practices. Leverages local and national CBO expertise to share best practices for HRSN social care navigation services. Develop and maintain an urban and rural outreach strategy for HRSN screening, navigation and HRSN care management services. Directly supports local CBOs to build sustainable capacity and provides tailored support in this area as needed. Regularly coordinates and collects feedback from social care navigators, garnering input on the salient challenges and barriers they are facing and collaborates around productive solutions. Creates and executes timely action plans for identified gaps or deficiencies in this area as needed. Responsible for managing social care navigator performance, including metric attainment. Provides performance reports (scorecards) to organizations providing social care navigation services in the SCN, utilizing data analysis techniques to convey opportunities for remediation and influence needed workflow changes. As necessary, facilitate training on network/CBO performance improvement and performance monitoring.
MINIMUM REQUIREMENTS
Education:
Bachelor's degree required and at least 3-5 years' experience managing projects. Experience with either screening, navigation, or case management activities, ideally with low-income or Medicaid individuals. Ability to work in a high pace environment and with the ambiguity & change that comes with a new program. Consistently utilizes critical thinking skills to resolve issues/challenges in workflows, generate process maps, and manages program performance. Ability to convey complex or technical information in a manner that others can understand, and ability to understand and interpret complex information from others. Results-driven and client-focused. Skilled with personal computer programs such as Microsoft Outlook, Microsoft Office (Word, Excel PowerPoint, etc.) Facilitation of meetings either in person or via virtual platforms such as Zoom.
WORK ENVIRONMENT
Care Compass Network utilizes a hybrid work model where employees are able to work remotely from a home office or from the CCN office, a non-clinical professional office setting. There may be some travel required for partner and/or network meetings.
