POSITION OVERVIEW
The Central Falls School District has partnered with the nationally recognized SMART Student Health and Wellness Model. The purpose of the collaboration is to break the cycle of poverty and positively impact the trajectory of lives. We achieve this by identifying the physical, social, emotional, and behavioral barriers to classroom success and then delivering interventions to address those barriers, thus creating optimal conditions for learning through the deployment of Active Access to deliver Active Care for ALL students, school faculty and staff, and students families. Each SMART Clinic has a full-time team of approximately 5 highly qualified clinical and support staff licensed to diagnose, treat, and prescribe for illness and injury at school. The school nurse teacher plays an integral role in this model. The CFSD seeks to hire a talented, innovative, caring School Nurse-Teacher to fill this full-time position.
DUTIES AND RESPONSIBILITIES
The following list of duties is not intended to be restrictive or all-inclusive.

Provide support in the delivery of quality clinical care.

Provide high-quality services to students, families, and staffCoordinate with the SMART Utilization Manager to ensure the deployment of SMART s signature Active Access approach.

Assist the medical providers when needed by delivering clinical services and implementing orders which may include providing medicines for common medical conditions, performing on-site blood draws, rapid testing, as well as a variety of immunizations and comprehensive screenings. Actively collaborate with the Behavior Health providers in the SMART Clinic.

Facilitate student referrals to external providers and conduct appropriate follow-up when needed.

Ensure all charting is completed after providing patient services, including EHR, eTHOS and Skyward.

Demonstrate clinical competency in school nursing practice and oversee student health care, providing first aid care and medically prescribed support and/or services.

Review medical records and ensure all patients' information is up to date, including their medical history and immunizations. Also, ensure patients are consented to. Complete student health screenings, such as ABC30 and PHQ2, PHQ9.Maintains confidential health records of students and other relevant health data.

Update all the required vaccine information and complete VFC documentation. Review and complete VFC forms when vaccines are received and during periodic checks.

Maintain logs sheets and inventories per school districts and clinic policies.

Close collaboration with SMART Clinic and school personnel in identifying and meeting students' social, emotional, and physical needs.

Perform an assessment of the students needs prior to dispensing medication and ensures all medical/pharmacy orders are current.

Ability to communicate professionally and collaboratively with administrators, service providers, students, and families on student health concernsIdentify opportunities for coordination with faculty on student health curriculum to maintain a healthy and safe environment.

Conducts classes on first aid, bloodborne pathogens, etc., to staff members.

Participate in professional development opportunities to supplement knowledge that will enhance student care.

Actively participate in the School and District Wellness & Staff Health & Safety Committee.

Any work deemed necessary by the building principal or building administrator.

The ideal candidate will demonstrate an enthusiasm for public health and a deep commitment to educating, informing, and advocating on behalf of students, staff, and parents regarding health, wellness, injury/illness prevention, and medical care, all within a state-of-the-art clinical setting.
