Texas Health Allen, 1105 Central Expwy North, Allen, TX 75013 Work hours: Part time; 3 days/week•8:30•5:00p Care Management Highlights Engaged leadership team Manageable case load Team that works together to ensure work/life balance for all Here's What You Need H.S. Diploma or Equivalent required Three years' relevant experience, preferably in a healthcare or service environment required Patient Care/ Customer Service / Healthcare experience strongly preferred What You Will Do Responsible for coordinating the clerical and administrative processes for entity care transitions management Responsible for managing departmental mailbox, phone lines and/or fax Supports transition planning activities as needed under the direction of care transition managers Calls vendors, physician offices, clinics, etc. Faxes (electronic and traditional) documents as needed Makes copies or gathers other data as needed Collect and enter data as needed Completes other tasks and projects assigned Additional perks of being a
