General Description:
Assists in the implementation of the students' transition planning process to include but not limited to: IEP development, data collection, coaching special education teachers and relevant personnel on best practices, facilitates and supports state compliance indicators, provides professional development on relevant transition planning topics as determined by the Department of Special Education. Coordinates work-based learning opportunities for students with disabilities including school based jobs and community events. Collaborates with school staff, community agencies and families to establish resources to best support student needs. Communicates and coordinates with community agencies to provide work-based learning opportunities for students with disabilities. Contacts are made regularly and frequently with instructional and administrative personnel as well as businesspersons and resource personnel outside of the school system. Complete state indicator reviews regarding transition.

Essential Job Functions :
Provides classroom consultation for special education teachers and relevant school special education staff in establishing work-based learning opportunities within the school and community. Provides support and assistance with implementation of transition related resources (work-based learning, self determination, community resources) as determined by the Department of Special Education. Provides assistance to special education teachers in identifying students strengths, preferences, interests, need(s)and assists with the application and synthesis of this information as part of transition planning. Individualizes development of skills to ensure compatibility between a student's strengths, preferences, interests and needs. Provides professional development opportunities to staff members on transition planning topics as determined by the Department of Special Education. Provides regular monitoring of the transition planning process for students. Coordinates agencies linkages with case managers from outside agencies, schools, students and families. Provides professional development to faculty and staff regarding transition services and compliance of transition tenets in the IEP. Responsible for working directly with students and teachers to coach, model and provide ongoing feedback to support postsecondary opportunities. Completes the calling of all graduates seniors of students with disabilities for Indicator 14 Surveys. Completes IEP reviews for Indicator 13. Performs other related duties as assigned.

Contacts:
Frequent contact with students, parents and teaching staff to provide consultation and assistance. Frequent contact with agency case workers, adult services agencies and businesses to communicate services and provide support.

Specific vision abilities required by this job include close vision, distance vision, depth perception and the ability to adjust focus.
Work Environment :
The employee regularly works in a school, office or business setting. Frequent travel is required within the school system and to employment sites with widely varied industrial settings. The noise level is quiet to moderate.
Qualifications :
Working knowledge of the: concepts, principles, and methodologies of instruction, learning theory and teaching methodologies for students with disabilities; of statutes, policies, guidelines and requirements related to special education; current practices in work experience for students with disabilities and of the principles and techniques of rehabilitative services. Some knowledge of state and local laws and regulations, including confidentiality and county geography.
Demonstrated ability to:
develop vocational goals for students and to plan, coordinate and deliver individualized resources and services both in the classroom and in the work environment; listen and communicate program goals to others effectively, both orally and in writing; collaborate with employers and with other instructional personnel; to provide constant attention and assistance to a number of children at the same time, especially children with special health conditions or disabilities and to perform physical tasks such as lifting children. Works well under stressful conditions or emergencies.
Demonstrated ability in:
decision making, problem analysis, conflict resolution, interpersonal relationships and planning; applying first aid procedures including CPR.
Additional Job Requirements:
Must hold a valid Virginia Collegiate Professional License or Post Graduate Professional License with an endorsement in at least one special education area. Master's Degree in Special Education preferred. A minimum of three years of teaching experience of students with disabilities is required. Valid Virginia driver's license.
