Job Description
Kitsap County is seeking two Case Management Specialist's to fill a critical role in supporting older adults, caregivers, and functionally impaired individuals. This position plays a vital role in ensuring access to essential services through programs such as Medicaid Alternative Care, Tailored Supports for Older Adults, and Family Caregiver, directly impacting the well-being and independence of vulnerable community members and supporting unpaid family caregivers. You will serve as a key resource for staff and partners, helping ensure smooth program operations and high-quality service delivery. This role is ideal for a skilled professional looking to deepen their expertise while making a measurable impact in the community. As the Case Management Professional, you will: Ensure the completion and accuracy of client files, documenting all case management activities in coordination with clients, families, service providers, and state and local agencies. Conduct comprehensive screenings and assessments and develop individualized care plans that identify client strengths, needs, and available support systems. Coordinate access to services by providing referrals and collaborating with interdisciplinary teams to ensure effective and appropriate service delivery. Manage client eligibility processes, including screening for program qualification and tracking presumptive eligibility status. Monitor program reports and track completion of new and ongoing case management tasks to ensure compliance with established deadlines. Generate and review data reports for multiple programs as requested to support program operations and oversight. Provide consultation and problem-solving support to staff as needed, particularly when the supervisor is unavailable.
What you bring:
Educational/Professional Experience Bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education in social work, Human Services, Psychology, or a related field AND two years of paid, on-the-job social service experience. OR A general bachelor's degree from a college or university accredited by an agency recognized by the U.S. Department of Education AND four years of paid, on-the-job social service experience.Technical/Industry Background:
Proficiency with state and federal program regulations affecting adults with functional impairments. Skilled in using assessment tools, care planning software, and maintaining accurate client documentation.Specialized Expertise:
Knowledge of local social services, healthcare systems, and community resources. Expertise in crisis intervention, discharge planning, and caregiver support strategies.Communication/Interpersonal Skills:
Strong interpersonal skills to engage with clients, families, and community partners. Excellent written and verbal communication for assessments, reports, and community presentations.Additional Preferred Qualifications:
Masters Degree in behavioral or health science Experience working with diverse populations and vulnerable adults. Demonstrated ability to provide innovative solutions in complex service coordination scenarios.For the complete job description:

Work hours for this position are typically Monday - Friday, 8 a.m. - 4:30 p.m. Driving Requirements The successful incumbent must meet the driving requirements of this position and comply with the Kitsap County Vehicle Use Policy. The successful incumbent must obtain a valid Washington State Driver's License prior to hire and maintain a satisfactory driving record as outlined in the Kitsap County Vehicle Use Policy.This position is categorized as:
Category 2 : Driver operates a personal vehicle as an essential function for official County business. Any employee using a personal vehicle on official County business shall be required to maintain auto liability insurance, in addition to having a valid driver's license.Criminal Conviction Standards :
