Comprehensive Community Services (CCS) Intake Coordinator / Care Coordinator Milwaukee Mental Health Associates, Inc. is seeking a CCS Intake Coordinator / Care Coordinator to lead the intake and enrollment process for Comprehensive Community Services (CCS). This role serves as the primary point of entry into the program, guiding referred individuals through screening, assessment, eligibility determination, and initial recovery planning. In addition to intake responsibilities, the position maintains a small care coordination caseload to support continuity of care and timely access to services.
Key Responsibilities:
  • Coordinate all aspects of the CCS intake and enrollment process, including assessments, functional screens, and initial recovery planning.
  • Facilitate timely and person-centered entry into services while ensuring compliance with DHS 36 and program timelines.
  • Maintain a small care coordination caseload to support ongoing service coordination, monitoring, and access to community resources.
  • Collaborate with individuals, families, providers, and internal staff to promote continuity of care.
Qualifications:
Bachelor's degree in Social Work, Psychology, or a related field required. At least one year of experience serving individuals with mental health, substance use, or co-occurring needs required, including CCS experience. Strong organizational, communication, and documentation skills are essential. Valid driver's license, reliable transportation, and current auto insurance are required.
