Kings View is seeking a Care Manager (Peer or Paraprofessional) to support Enhanced Care Management (ECM) and Community Supports (CS) programs. This role works directly with members and their support systems to coordinate care, address gaps in services, and promote whole-person wellness. The Care Manager partners with a multidisciplinary care team to ensure members receive coordinated services across medical, behavioral health, and social support systems, including addressing social determinants of health.Key Responsibilities:
Care Coordination & Case Management:
Conduct outreach, engagement, and care coordination for members Complete assessments and develop individualized care plans Implement and monitor care plans to ensure member needs are met Coordinate with healthcare providers, social services, and community resources Prevent duplication of services and ensure seamless care deliveryMember Support & Advocacy:
Engage members in services and support their recovery and wellness goals Advocate for members with healthcare providers and service agencies Accompany members to appointments as needed Provide education on health promotion and self-management Monitor treatment adherence, including medicationsCommunity & Resource Navigation:
Connect members to housing, transportation, benefits, and other supports Assist with access to services such as SSI, medical care, and veteran benefits Coordinate discharge planning with hospitals and providers Support members in accessing appropriate levels of careCrisis Awareness & Safety:
Utilize trauma-informed care, motivational interviewing, and harm reduction approaches Respond appropriately to high-risk situations and behavioral health crises Follow protocols and coordinate with emergency services when neededGeneral Duties:
Maintain accurate documentation and case records Collaborate with multidisciplinary teams Work flexible hours including occasional evenings and weekends Perform other duties as assignedWhat You'll Bring:
Required Qualifications:
High School Diploma or equivalent One (1) year of relevant experience Valid California Driver's License (Class C)Required Experience:
Experience working with one or more of the following populations: homeless individuals, individuals with serious mental illness (SMI), substance use disorders (SUD), or involvement in the criminal justice systemRequired Skills:
Strong communication and interpersonal skills Ability to conduct assessments and develop care plans Ability to coordinate services and navigate community resources Strong organizational and documentation skills Ability to work independently and collaborativelyPreferred Qualifications:
Bachelor's Degree (BA/BS) Bilingual English/Spanish CNA or Medical Assistant experienceProfessional Attributes:
