Job Description
Under the supervision of the therapist, the Paraprofessional is responsible for direct face to face contact, behavioral intervention and relational social skill building to adults, children and adolescents in the community, as well as the school and home settings. The paraprofessional will provide input and work together with the therapists to create, update and implement treatment plans for all consumers on their assigned caseload. The Paraprofessional will also meet weekly with the Treatment Team to discuss consumer progress.
Duties and Responsibilities:
1. Complete, submit and verifies accuracy of documentation, including but not limited to BIRP progress notes, intake paperwork, etc., while maintaining documentation guidelines set forth by the state 2. Attends weekly/monthly meetings and maintains effective working relationships with the Therapists, School Administrators, and school personnel 3. Establishes and maintains effective working relationships with Clinical Director, Therapists, families and school personnel in order to engage in effective communication. 4. Intervenes in crisis in a calm, appropriate manner that maintains staff and consumer safety 5. Works effectively with consumers providing individualized services and co-facilitates with the Therapists, including but not limited to planning and implementing program extensions. 6. Individual must have a lap top computer that has office word program and is internet accessible; and reliable transportation to provide job functions. Notenetic EHR/EMR is the system used at agency. 7.Services provided:
Adult Case Management; C&A Community Support Individual; Adult PSR-Psycho-social Rehabilitation; Skills Training; and Group Training. 8. Completing Prior Authorizations for support services in MMIS and Provider Connect Portals 9. This position is entirely out in the field/ seeing clients out in the community at home or in school settings.Benefits:
