Position Summary The Paraprofessional provides direct support services to individuals receiving behavioral health and community-based services under the supervision of a Qualified Professional. This role assists individuals with skill development, community integration, social engagement, and the achievement of goals identified in their Person-Centered Plan. Services are provided primarily in community settings and may include assistance with daily living skills, social skills development, community participation, and independent living supports. Essential Responsibilities Provide direct support services in accordance with each individual's Person-Centered Plan. Assist individuals with skill-building activities that promote independence and self-sufficiency. Support community integration, social engagement, and participation in meaningful activities. Assist with activities of daily living and other individualized support needs. Monitor and report significant changes in behavior, health status, or environmental concerns. Maintain accurate and timely service documentation in accordance with agency policies and Medicaid requirements. Promote individual choice, dignity, self-determination, and recovery-oriented practices. Ensure the health, safety, and well-being of individuals served. Participate in supervision meetings, staff training, and team collaboration activities. Maintain confidentiality and professional boundaries at all times. Minimum Qualifications Required High School Diploma or GED. At least 18 years of age. Valid North Carolina Driver's License. Reliable transportation. Ability to pass required background screenings and meet agency employment requirements. Strong communication and interpersonal skills. Ability to work independently and professionally within community settings. Preferred Coursework in Human Services, Psychology, Social Work, Counseling, or a related field. Experience supporting individuals with mental health, intellectual/developmental disabilities, or substance use challenges. Experience providing direct support services in community-based settings. Personal or lived experience that contributes to understanding recovery, resilience, and community support services.
