Job Description
In Home Modification Assessments PT/OT We are looking for an experienced OT or PT who can provide 5-10 Home Modification and/or Assistive Technology Assessment visits per week. Must be willing to travel in a thirty mile radius for in home assessments in the Altoona area. This is a per diem position on an Independent Contractor basis. Therapy Choice is owned and operated by a Physical Therapist and an Occupational Therapist who have been working to keep our seniors living at home independently for over twenty years. Our Home Modification Assessment is the tool used to fund/expedite needed modifications to keep our seniors at home. www.therapychoice.net
