Position Overview The Illinois Department of Public Health is seeking a highly motivated detailed oriented individual to conduct inspections, investigations and surveys of Home Service and Placement and Home Nursing and Placement Agencies for the Division of Health Care Facilities and Programs to ensure compliance with the new state licensure rules and requirements "77 Illinois Administrative Code, Part 245 Home Health, Home Nursing and Home Services Agencies".Benefits Statement As a State of Illinois Employee, you will receive a robust benefit package that includes the following: Flexible and hybrid work schedules are available in many program areas (when available and dependent upon position) Competitive Group Insurance benefits including health, life, dental and vision plans. For more information about our benefits please follow this link: https://www2.illinois.gov/cms/benefits/Pages/default.aspxEssential Functions Travels and conducts surveys of Home Nursing Placement and Home Services and Placement Agencies and other related services a required to determine the level of compliance with State Licensure rules and requirements. Prepares licensure survey forms, memoranda and reports of findings as required. Investigate complaints as assigned. Conducts and participates in on-site visits, pre-planning, entrance and exit conferences. Participates in staff meetings, training sessions, seminars and workshops designed for improvement in survey process and individual performance. Performs other duties as required or assigned which are reasonably within the scope of the duties enumerated above. Minimum Qualifications Requires (a) graduation from an approved nursing education program resulting in an Associate or a Diploma Degree in Nursing and three (3) years of professional nursing experience or, (b) a Bachelor's Degree in Nursing and two (2) years of professional nursing experience or, (c) a Master's Degree in Nursing. Requires current licensure as a Registered Nurse in the State of Illinois. Preferred Qualifications One (1) year experience in a Community Nursing setting. Three (3) years professional nursing experience. Two (2) years experience participating in the process for completion of all survey functions including but not limited to annual recertification, licensure, follow-ups, complaints and incident investigation surveys. One (1) year experience preparing complex written and oral reports. One (1) year work experience in Microsoft Word, processing, and spreadsheet applications (i.e., Microsoft Word, Microsoft Outlook, Excel, PowerPoint). Ability to exercise judgement, discretion and maintain confidentiality during the conduct of the survey process. Conditions of Employment Requires physical, visual, and auditory ability to carry out assigned duties. Requires ability to travel in the performance of duties. Requires appropriate, valid driver's license. Certain provisions of the revolving door restrictions contained in 5 ILCS 430/5-45 apply to this position. As a result, the employee should be aware that if offered non-State employment during state employment or within one year immediately after ending State employment, the employee shall, prior to accepting any such non-State employment offer, notify the Office of the Executive Inspector General for the Agencies of the Illinois Governor (OEIG) or may be subject to a fine. This position has been found to meet the requirements in section 4A-101 of the Illinois Governmental Ethics Act requiring the occupant/employee to file a Statement of Economic Interest. The Illinois Governmental Ethics Act (5 ILCS 420/4A et seq.) requires certain state officials and employees to file annual Statements of Economic Interest with the Office of the Secretary of State that will be made available for examination and copying by the public. All employees required to file a Statement of Economic Interest are also required to file the Supplemental Statement of Economic Interest with Executive Ethics Commission.
