Job Description
The mission of the Louisiana Department of Health is to protect and promote health and to ensure access to medical, preventive, and rehabilitative services for all residents of the State of Louisiana. Join the Louisiana Department of Health's Office of Rural Health Transformation and Sustainability and help improve healthcare access and outcomes for rural and underserved communities across the state. In this dynamic role, you'll support initiatives that strengthen local health systems, promote integrated and efficient care, and advance healthy lifestyles. With oversight of over $250 million annually in federal and state grant programs, this is an exciting opportunity to lead transformative efforts, drive data-informed decisions, and make a measurable impact on the health and well-being of Louisiana residents. Seven years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR Six years of full-time work experience in any field plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR A bachelor's degree plus four years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services; OR An advanced degree or a Juris Doctorate plus three years of experience in developing, managing, or evaluating health or social service programs; or in public health, public relations, social services, health services, health regulation, or administrative services.

EXPERIENCE SUBSTITUTION
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field. - Develops current and long-range plans, policies, and procedures to implement workforce and sustainable access initiatives under the Rural Health Transformation Program grant.
- Plans, develops, and supports the execution of strategies and initiatives, ensuring timelines, budget milestones, sub-grantee reporting, and deliverable tracking are met.
- Ensures compliance with CMS regulations and verifies that sub-grantees align with workforce and sustainable access program goals.
- Prepares and submits workforce and sustainable access grant reports for inclusion in CMS reporting and provides resource materials for Advisory Council and public engagement.
- Conducts research on rural health workforce models at state and national levels and advises leadership on best practices and updated strategies.
- Provides ongoing consultation and technical assistance to sub-grantees and potential grantees through webinars, on-site visits, and other engagement activities.
- Develops and implements training programs for workforce and sustainable access grant submission, monitoring, and continued program updates.
- Serves as the contract monitor for approximately 60 sub-grantees, ensuring milestones, data metrics, and funding criteria are met.
- Collaborates with internal LDH teams, Medicaid and Health Standards Sections, and other state agencies to develop rules, regulations, and strategies supporting workforce initiatives.
- Coordinates activities among internal teams, community partners, and external stakeholders to ensure successful implementation, monitoring, and sustainability of workforce and access projects.
