Monday - Friday, 8:30 AM - 4:30 PM The ESG Homeless Case Manager provides comprehensive case management services to homeless individuals and families, supporting them in securing and maintaining safe, affordable housing and accessing supportive services. ESG Homeless Case Manager Key Responsibilities Determine program eligibility and complete needs assessments Develop goal-oriented service plans with clients and conduct regular follow-ups and annual assessments Provide case management, referrals, and coordination with community resources Assist with housing search, inspections, landlord coordination, and lease agreements Ensure housing meets program requirements, Fair Market Rent, and rent reasonableness Conduct home visits, housing inspections, and ongoing client check-ins Maintain accurate and timely case files, documentation, reports, and HMIS entries Support clients with life skills, budgeting, transportation, and housing stability Collaborate with housing staff, landlords, and community partners Maintain confidentiality and professionalism at all times ESG Homeless Case Manager Qualifications Minimum of one year of case management experience or related training Strong organizational, communication, and computer skills (MS Word, Excel, agency databases) Knowledge of local social service networks and community resources Ability to work independently and as part of a team Valid PA driver's license, reliable transportation, required clearances, and vehicle insurance Willingness to travel and transport clients using a personal vehicle Additional DetailsDriver's License:
