Job Description

Job Overview We are seeking a dynamic and compassionate Family Nurse Practitioner (FNP) to join our dedicated healthcare team. In this vital role, you will provide comprehensive primary care services to diverse patient populations across various settings, including outpatient clinics, urgent care, and post-acute care environments. Your expertise will help promote health, prevent illness, and manage chronic conditions with a focus on patient-centered care. This position offers an exciting opportunity to make a meaningful impact on individuals and families while working in a collaborative, innovative healthcare environment. Duties Conduct thorough physical examinations and health assessments for patients of all ages, including pediatrics, geriatrics, and adult populations. Develop and implement personalized treatment plans, including medication administration, injections, IV infusions, and wound suturing when necessary. Manage acute and chronic illnesses by utilizing advanced clinical skills such as disease diagnosis, medication management, and health coaching. Collaborate with multidisciplinary teams to coordinate patient care, discharge planning, and case management across inpatient, outpatient, and home care settings. Utilize electronic health record (EHR) systems such as Epic or eClinicalWorks to document medical records accurately and efficiently. Perform diagnostic evaluations using laboratory testing, sonography, medical imaging, and specimen collection/processing techniques. Provide triage services via telehealth or in-person consultations to assess patient needs promptly and direct appropriate level of care. Requirements Valid Family Nurse Practitioner certification/license with current state licensure. Proven experience in primary care settings with familiarity in medical office procedures and documentation standards. Strong clinical skills including physical examinations, vital signs assessment, anatomy knowledge, and physiology understanding. Experience with EMR/EHR systems such as Epic or Cerner is highly preferred. Knowledge of CPT coding, ICD coding (including
ICD-9/10
), and utilization review processes. Ability to perform procedures such as suturing, catheterization, spinal taps, phlebotomy, sterile processing techniques, and dermal fillers. Experience working with diverse populations including children, seniors requiring memory or dementia care, hospice & palliative medicine patients or those with developmental disabilities. Critical care experience in ICU or Level I/II trauma centers is advantageous; familiarity with emergency medicine protocols is a plus. Skills in infection control practices and acute pain management strategies are essential. Strong communication skills for patient education and collaboration within multidisciplinary teams.
