Duties Conduct detailed physical examinations and health assessments for patients across all age groups, including pediatrics, geriatrics, and adult populations. Develop and implement individualized care plans based on clinical evaluations, diagnostic results, and patient needs. Manage acute illnesses and chronic conditions with evidence-based interventions, including medication administration and lifestyle counseling. Utilize electronic health record (EHR) systems such as Epic, eClinicalWorks, or Cerner to document patient encounters accurately and efficiently. Perform specialized procedures such as suturing, IV infusions, catheterization, and dermal fillers when appropriate. Coordinate with multidisciplinary teams for case management, discharge planning, and referrals to specialists like gastroenterology or infectious disease care providers. Engage in telehealth consultations to expand access to care while ensuring compliance with HIPAA regulations and medical documentation standards. Qualifications Valid Family Nurse Practitioner certification with current state licensure. Proven experience in primary care settings with proficiency in medical office operations and EMR/EHR systems. Strong knowledge of physiology, anatomy, pharmacology, and medical terminology essential for accurate patient assessment and diagnosis. Experience with diverse clinical procedures including venipuncture, sterile processing techniques, and basic life support (BLS). Familiarity with complex medical conditions such as critical care scenarios, ICU or trauma center experience preferred. Ability to perform triage effectively in urgent care or emergency room environments; experience with hospital medicine or post-acute care is advantageous. Excellent communication skills to educate patients on health coaching, medication management, family planning, and nutrition strategies.
