Position:
Chart Auditor The Chart Auditor is responsible for reviewing and evaluating medical records to ensure documentation accuracy, regulatory compliance, and billing integrity. This role supports both clinical and administrative teams by identifying discrepancies, improving record-keeping practices, and helping maintain the clinic's high operational standards. Key ResponsibilitiesDocumentation Review & Compliance Audit patient charts to ensure completeness, accuracy, and consistency with clinical standards Verify that medical documentation supports services rendered and aligns with billing practices Identify missing, incomplete, or inconsistent entries Ensure compliance with internal documentation protocols Billing Accuracy Cross-reference chart documentation with invoices to ensure billing integrity Report discrepancies to the Finance Manager in a timely manner Collaboration & Reporting Prepare audit summaries highlighting trends, deficiencies, and areas for improvement Collaborate with owners, veterinarians, and technicians on charting best practices Administrative Support & Expectations Maintain organized audit records and documentation logs Assist with policy development related to medical record documentation Handle medical records with a high level of professionalism and confidentiality Demonstrate strong attention to detail and a proactive approach to problem-solving Foster a culture of accountability, compliance, and continuous improvement Communicate findings with clarity, empathy, and respect Qualifications Experience in veterinary medicine, medical records auditing, or healthcare documentation preferred Strong understanding of medical terminology (veterinary experience a plus) High attention to detail and organizational skills Ability to work independently and collaborate with a team Proficiency in practice management software and basic computer skills Why Join Southpaw?
