The ideal candidate will develop and maintain referral relationships with physicians, hospitals, rehabilitation facilities, assisted living facilities, case managers, and community organizations to generate qualified referrals and support agency growth. This position requires strong communication skills, healthcare sales experience, and the ability to build long-term professional relationships while maintaining compliance with all Medicare, Medicaid, and home health regulations. Responsibilities Develop and maintain referral sources including physicians, hospitals, skilled nursing facilities, assisted living facilities, case managers, discharge planners, and community organizations. Conduct regular face-to-face sales calls and presentations to referral partners. Educate referral sources on Capstone Home Health services and admission criteria. Identify new business opportunities and develop strategies to increase referrals. Track referral activity and maintain accurate records in CRM systems. Attend networking events, healthcare meetings, and community outreach activities. Collaborate with intake, clinical, and administrative teams to ensure timely referral processing. Monitor market trends and competitor activities. Maintain knowledge of Medicare, Medicaid, managed care, and home health regulations. Meet or exceed established referral and growth goals. Represent the agency professionally in the community. Qualifications Home health, hospice, hospital, or healthcare marketing experience strongly preferred. Established relationships within the Jacksonville healthcare community preferred. Excellent verbal, written, and presentation skills. Strong organizational and time-management abilities. Ability to work independently and meet performance goals. Proficiency with Microsoft Office and CRM software. Valid driver's license, reliable transportation, and automobile insurance.
