Saturdays Job Summary:
Ochoa's Consulting has the privilege of supporting and guiding families through the programs we offer at our office, within communities, support groups, schools, and other community settings where we work collaboratively. Our purpose is to provide education, resources and support so that families can better understand systems that are often complex. Through this work, we aim to strengthen their knowledge and contribute to an improved quality of life for their loved ones with disabilities. The Independent Facilitator supports the participant in navigating the SDP to achieve a more self-directed life. They are responsible for guiding the person-centered planning process, developing an individualized budget, and advocating for the the participant's needs and goals.
Key Responsibilities:
Person-Centered Planning (PCP): Lead or participate in the PCP process to identify the participant's hopes, dreams, and long-term goals.
Budget Management:
Help develop and manage the Individual Budget and Spending Plan, ensuring funds are allocated to meet the participants goals.
Service Coordination:
Locate and negotiate rates with service providers, community resources, and staff that are not part of the traditional regional center vendor list.
System Navigation:
Assist with the administrative complexities of the SDP, including paperwork for the regional center to the planning process.
Advocacy:
Attend individual Program Plan (IPP) and (IEP) meetings to advocate for the participant's vision and ensure their voice remains central to the planning process.
Qualifications & Requirements:
Training:
Preferred Skills:
Strong active listening, negotiation and \"out-of-the-box\" problem-solving skills.
Knowledge:
Familiarity with California's Lanterman Act and Regional Center Procedures is highly preferred.
