Job Description
Schedule:
Full-Time | 3:00pm-11:00pm Shift 5 shifts per week Every other weekend required Position Overview Jewish Community Housing Corporation (JCHC) is seeking a compassionate and dependable Full-Time Home Health Aide (HHA) / Personal Care Assistant (PCA) to provide high-quality, resident-centered care in our assisted living community. This role supports residents overnight by delivering personal, clinical, nutritional, and restorative care in accordance with individualized care plans and all applicable federal, state, and local regulations. The HHA/PCA reports directly to the Director of Wellness and plays a vital role in ensuring resident safety, comfort, and dignity during overnight hours. Essential Duties & Responsibilities Provide direct personal care and assistance with Activities of Daily Living (ADLs) per Individualized Service Plans (ISP) or Health Service Plans (HSP) Monitor residents overnight and promptly report any changes in condition to the licensed nurse Obtain and document vital signs, meal intake, and urine output as required Accurately document care and ADLs in flow sheets and restorative records Recognize emergency situations and respond appropriately Report incidents, injuries, and suspected abuse immediately in accordance with regulations and company policy Partner with licensed nursing staff to support restorative therapy programs Assist with maintaining and cleaning medical, adaptive, and assistive equipment Maintain compliance with all federal, state, and local resident care regulations Participate in care planning, team meetings, required trainings, and annual in-services Demonstrate JCHC's Mission, Vision, and Values in all resident and team interactions Qualifications Valid New Jersey HHA or CNA certification (required) High School Diploma or GED (required) Previous experience as an HHA, PCA, or CNA preferred Experience working with seniors or in assisted living preferred Ability to work overnight shifts and every other weekend Strong communication, documentation, and time management skills Ability to make independent decisions and exercise sound judgment Ability to remain calm and professional in emergency or high-pressure situations Must be reliable, compassionate, and team-oriented Physical Requirements Frequently assist/support residents up to 50 lbs Occasionally assist/support up to 150 lbs Frequently lift/carry up to 40 lbs Occasionally lift/carry up to 70 lbs Ability to lift up to 3 feet and carry up to 10 yards Must be able to stand, walk, bend, and assist residents throughout the shift Training & Team Engagement Participate in onboarding and required regulatory training Attend regular meetings and annual in-services Actively contribute to resident engagement and team success Support a positive, respectful, and compliant care environment Equal Opportunity Employer Jewish Community Housing Corporation is an Equal Opportunity Employer .
