Full-Time Registered Nurse (RN) Home Care Assessments

Anchor Health Homecare Services Overview We are hiring a Full-Time RN to perform in-home patient assessments across our service area. This role offers a steady caseload with the independence of field work and the support of an experienced clinical team. If you enjoy autonomy but still want stability, this is a strong fit. What You'll Do Conduct in-home nursing assessments (initial evaluations, reassessments, and PRN visits) Evaluate patient condition including physical, cognitive, and psychosocial status Review medications and assess safety risks and fall risk Complete documentation within: 48 hours (initial & PRN visits) 5 days (reassessments) Collaborate with physicians and care team to ensure continuity of care What You Need Active RN license (in-state) Minimum 1 year of nursing experience Home health or case management experience preferred Reliable transportation and valid driver's license Strong time management and documentation skills What We Offer Competitive compensation Consistent, full-time caseload Weekly direct deposit Supportive clinical leadership Flexible scheduling within a full-time structure to join a team that values independence, flexibility, and clinical excellence.

$110,000.00 - $120,000.00 per year
