Job Description

Job Overview The Swing Bed Coordinator is an RN/LPN or Case Manager with direct Swing Bed program experience who has full operational ownership of the hospital's Swing Bed services. This role is responsible for daily census management, CMS compliance, skilled documentation oversight, referral intake, interdisciplinary coordination, and financial performance of the Swing Bed program. The Swing Bed Coordinator serves as the clinical and operational authority for program admissions, patient progression, and discharge planning, ensuring that every Swing Bed stay meets Medicare skilled criteria, is fully supported by documentation, and progresses efficiently toward discharge. Essential Responsibilities Swing Bed Operations & Census Control Own daily Swing Bed census, bed availability, and admission prioritization Review and approve Swing Bed admissions using CMS skilled care criteria Monitor patient progression, length of stay, and discharge readiness Serve as the primary point of contact for all Swing Bed activity Regulatory Compliance & Documentation Accuracy Ensure compliance with CMS Swing Bed Conditions of Participation Perform daily reviews of skilled nursing, therapy, and provider documentation Identify and correct documentation gaps in real time Maintain survey-ready documentation standards Referral Intake & Development Manage inbound Swing Bed referrals and acceptance decisions Maintain referral, bed availability, and conversion tracking Participate in referral outreach and relationship development Interdisciplinary Clinical Collaboration Ensure nursing skilled documentation discipline Coordinate timely therapy evaluations Partner with providers on skilled necessity documentation Facilitate interdisciplinary communication for care progression and discharge Financial & Performance Oversight Monitor census trends and average length of stay Support accurate charge capture and billing workflows Participate in denial prevention and documentation audits Report performance metrics to leadership Program Development & Sustainability Maintain Swing Bed policies, workflows, and admission criteria Support staff education on Swing Bed regulatory requirements Assist with diagnosis expansion and pathway development Required Qualifications Active, unrestricted RN license, LPN License and Case Manager Experience Minimum of 3 years direct Swing Bed experience Strong knowledge of Medicare skilled nursing criteria and Swing Bed Conditions of Participation Experience in acute care, med-surg, case management, or utilization review Ability to work independently with strong organizational skills Performance Expectations Maintain compliant, audit-ready skilled documentation Achieve and sustain projected daily Swing Bed census Ensure timely therapy evaluations Reduce documentation-related denials Demonstrate effective referral conversion and patient throughput
