Long-Term Care Key Responsibilities:

Conduct comprehensive health assessments, including physical exams and medication reviews Develop and manage individualized care plans in collaboration with an interdisciplinary team Manage chronic and acute conditions, providing appropriate care and referrals Order and interpret diagnostic tests (labs, imaging, etc.) Prescribe medications within scope and regulatory guidelines Participate in chronic care management service planning Educate residents and families on health maintenance and prevention Attend interdisciplinary meetings and contribute to quality improvement Maintain accurate EMR documentation

Qualifications:

Master's degree in Nursing and completion of an accredited NP program Unrestricted New York Nurse Practitioner license DEA license to prescribe medications Eligible for Medicare and Medicaid provider status Minimum 3 years of clinical experience (preferably in geriatrics or long-term care) Experience with eClinicalWorks (eCW) preferred
