This full-time opportunity offers a stable weekday schedule, clinical autonomy, and the ability to build meaningful relationships with patients while improving outcomes and reducing unnecessary hospitalizations. The provider will deliver in-person care within Skilled Nursing and Post-Acute Care facilities while collaborating closely with physicians, nursing staff, and interdisciplinary care teams.
