Job Description

Job Description Responsible for providing skilled nursing, intervention and supportive care to patients in the home setting for evening, night, weekend and holiday on-call duty in accordance with the plan of care. Compensation Paid at a salaried rate. Attendance at scheduled staff meetings, orientation or training sessions, or other assigned duties will be paid at the agreed upon hourly rate. Mileage accrued through travel required for the job will be reimbursed at the standard agency rate. Work Hours and Frequency The on-call nurse will be available by phone to meet the needs of the patients, as assigned for two 24 hour periods beginning at 8:00 am Saturdays and Sundays. He /She is responsible for identifying and coordinating patient/family care to support patients with advanced illness and families in home, skilled nursing facility or residential care. Facility. Communication related to on-call interventions will be documented and communicated to the DON with his/her assessment, plan of care changes and on-going need. Qualifications Graduate of an accredited school of Nursing. Valid license as a Registered Nurse in the state providing nursing care. Hospice or Home Health experience is preferred. Minimum one year recent professional nursing experience Valid Driver's License Computer skills
Knowledge of:
Nursing techniques and procedures. Medical terminology. Medication use and effects. Medical record keeping practices.
Ability to:
Interpret orders and carry out instructions while exercising judgment within the scope of the position and established nursing practices. Work with limited supervision in an independent setting, while able to recognize situations in which supervision and guidance are required. Communicate therapeutically with the patient, family and public and maintain a compassionate, empathetic attitude. Respond to all calls with urgency. Knowledge and Skills Necessary to

HIPPA Compliance Physical Demands:
Intermittent physical activity including walking, standing, sitting, lifting and supporting patients. Staff may be exposed to virus, disease and infection from patients and specimens in working environment. Staff will be required to work in patient's homes or where the patient calls home and be responsible for own transportation. Ability to turn, reposition, and assist patients to transfer and ambulate Must be able to bend, stoop, push and pull. Must be able to lift a minimum of 25 pounds. Responsibilities The Registered Nurse is required to provide point of service electronic charting and documentation at time of patient visit or admissions Must be able to operate a computer and or IPAD, Responds to answering service within 5 minutes and be ready to travel in 10-15 minutes. Conducts admissions as necessary. Conducts physical and psychosocial assessments of patient's and family's needs after hours and determines intervention. Provides skilled nursing care focusing on pain control and symptom management. Provides interim guidance to the patient and his or her family regarding diagnosis medications, treatments and patient progress. Documents care in a thorough and timely manner. Works closely with IDG to provide continuity of care and support for the patient and family. Collaborates with vendors to obtain DME supplies and medication. Collaborates with hospitals, SNF, AFH or any assisted living facility to adjust interventions and plan of care as well as provide teaching/training to unlicensed personnel or family members. Operates under the supervision of the after-hours supervisor and under physician orders. Operate within the framework of policies and procedures, state and federal regulations and the patient's plan of care. Will be available by phone to meet the needs of the patients, as assigned Assume primary responsibility for census during assigned shift. Maintain regular communication with agency staff and the patient's referring physician and/or case manager concerning patient/family care needs and response to interventions.
