CLINICAL LEADER (DON / HWD) Charter Senior Living of Hasmer is seeking an experienced and compassionate Director of Nursing / Health & Wellness Director to lead clinical operations within our assisted living and memory care community. POSITION SUMMARY

The Director of Nursing / Health & Wellness Director is responsible for overseeing all clinical operations within the community, ensuring high-quality resident care, regulatory compliance, and strong team leadership. This role partners closely with the Executive Director and department leaders to support operational excellence and a positive culture.

KEY RESPONSIBILITIES

Clinical Leadership and Resident Care Oversee all nursing services across Assisted Living and Memory Care Complete resident assessments, care plans, and service plan reviews Monitor resident health status and coordinate physician communication Ensure accurate medication management and clinical documentation Lead quality initiatives including fall prevention and infection control Team Leadership and Development Supervise nurses, caregivers, and medication aides Provide coaching, training, and ongoing performance development Support staff retention and engagement initiatives Assist with staffing coordination and scheduling oversight Promote a positive, team-oriented culture aligned with Charter values Compliance and Quality Assurance Maintain compliance with state regulations and survey readiness Lead incident reviews, investigations, and follow-up actions Implement and monitor clinical policies and procedures Partner with leadership to ensure operational and clinical alignment

QUALIFICATIONS

Active LPN or RN license in the state of Wisconsin required Previous leadership experience such as Director of Nursing, Health and Wellness Director, Resident Care Director, or Clinical Manager Experience in senior living, assisted living, long-term care, or skilled nursing preferred Strong knowledge of state regulations and clinical compliance Excellent leadership, communication, and problem-solving skills Passion for serving the senior population
