This is a great opportunity for those passionate about helping others navigate medical, behavioral health, and social support systems. Are you a Credentialed Alcoholism and Substance Abuse Counselor (CASAC)? CASACs with at least two years of human services experience are strongly encouraged to apply.

Key Responsibilities:

Work collaboratively with participants to assess health goals and implement individualized Plans of Care. Complete HARP (Health and Recovery Plan) assessments to connect members with specialized community-based services. Support members in overcoming barriers to healthcare by providing health literacy education and addressing social determinants of health. Assist high-risk members in managing chronic behavioral health, medical, and substance use conditions. Link participants to appropriate health, behavioral health, and community resources. Coordinate with healthcare providers and community agencies to promote primary and preventive care. Assist participants in reducing or eliminating avoidable hospital and emergency room usage. Maintain and manage all aspects of participants Electronic Health Records (EHR). Promote a culture of inclusion, belonging, and respect.

Qualifications:

Bachelor's degree in any field with at least three years of human services experience , OR Master's degree in a human services field with at least one year of experience , OR CASAC certification with two years of human services experience . Proficiency in Microsoft Office. Prior experience with the Medicaid delivery system preferred but not required. Reliable transportation and a valid New York State driver's license required. Ability to work effectively in a fast-paced environment.
