$80,000.00 - $85,000.00 per year About the Role We are seeking a dedicated, compassionate, and autonomous Health Home Registered Nurse (RN) Case Manager to join our mobile care team. In this role, you will provide comprehensive, patient-centered care and case management to individuals within their homes and communities throughout the Bingham Farms operating area. Because this is an "on the road" position with no consistent starting location, you will enjoy a high degree of independence while serving as the vital link between patients, their families, and the broader healthcare system.
Key Responsibilities Clinical Assessment & Care Planning:
Conduct comprehensive initial and ongoing assessments of patients' physical, emotional, and social needs. Develop, implement, and update individualized plans of care.
Case Management & Coordination:
Coordinate care across multi-disciplinary teams, including physicians, specialists, therapists, and community resources to ensure seamless service delivery.
Patient Advocacy & Education:
Educate patients and their caregivers on chronic disease management, medication adherence, post-acute care instructions, and preventative health practices.
Medication Management:
Reconcile medications, administer prescribed treatments, and monitor patients for adverse reactions or changes in clinical status.
Accurate Documentation:
Maintain timely, accurate, and compliant electronic health record (EHR) documentation in accordance with state, federal, and agency guidelines.
Crisis Intervention:
Identify early signs of health regression or social instability and intervene proactively to prevent unnecessary hospital readmissions or emergency room visits.
Qualifications & Requirements License:
Active, unrestricted Registered Nurse (RN) license in the State of Michigan.
Education:
Associate Degree in Nursing (ADN) required; Bachelor of Science in Nursing (BSN) preferred.
Experience:
Minimum of 1-2 years of nursing experience; experience in home health, community health, hospice, or case management is highly preferred.
Travel:
Must possess a valid driver's license, a reliable vehicle, and clean driving record, as this position requires daily travel to patient residences.
Skills:
Strong clinical judgment, excellent communication skills, adaptability, and proficiency with digital charting/EHR platforms.
Certifications:
Current BLS/CPR certification.
