Job Description
Full Time Monday - Friday 8:00 a.m. to 4:30 p.m. Position Summary The HIM Clerk supports the Health Information Management (HIM) Department by maintaining accurate, complete, and accessible medical records in compliance with federal and state regulations, hospital policies, and Release of Information (ROI) guidelines. This position is responsible for filing, retrieving, scanning, assembling, analyzing, and tracking medical records in both electronic and paper formats. The HIM Clerk ensures timely access to patient information while safeguarding confidentiality and supporting efficient hospital operations in a rural Critical Access Hospital environment. Primary Duties Confidentiality & Compliance Maintains strict confidentiality in accordance with HIPAA Privacy and Security regulations and hospital policies. Treats internal and external coworkers, providers, and stakeholders with professionalism and respect. Complies with all guidelines related to Release of Information (ROI) and patient record access. Medical Record Maintenance & Management Performs daily scanning of patient information into medical records in accordance with HIM policies and procedures. Analyzes records for completeness and deficiencies, including required signatures, test results, and supporting documentation. Performs chart review in EMR systems and assists with chart routing and transfer processes as needed. Communicates with providers regarding incomplete records, required signatures, and documentation deficiencies while following established policies and procedures. Release of Information (ROI) Processes ROI requests by verifying authorization validity and ensuring compliance with regulatory requirements. Maintains ROI logs and documentation. Pulls, copies, faxes, prepares billing statements, and mails medical records as appropriate. Responds to phone calls, faxes, and requests related to patient information and medical records. Record Tracking & Department Support Prepares and distributes weekly and monthly delinquent record lists to appropriate providers. Distributes incoming faxes and ensures proper routing and filing. Assists with unit consolidation, physician billing processes, backlog record research, and census reporting as needed. Performs weekly processes related to expired patient records. Utilizes computer systems to track Living Will and Durable Power of Attorney (DPOA) documentation. Assists with organizing and maintaining departmental and medical staff documentation, files, spreadsheets, and meeting materials. Assists with provider affiliation verification, reappointment correspondence, and related medical staff tracking processes. General Duties Provides support to HIM coworkers during periods of increased workload or backlog. Serves as backup support for department administrative functions as needed. Performs additional duties as assigned by the Director of Revenue Cycle. Requirements Education High School Diploma or GED equivalent required. A technical certificate or associate's degree in related field, preferred. Experience Basic computer skills Basic knowledge of medical terminology and health records management. Familiarity with electronic health record (EHR) systems. Strong organizational skills and attention to detail. Ability to maintain confidentiality and adhere to HIPAA regulations. Language Skills Must be capable of communicating clearly (reading, writing, and speaking) in the English Language. Must be capable of reading and comprehending written policies/procedures and instructions. Must have the ability to receive and relay phone or other business messages. Equipment Must be able to operate computer, telephone and other department specific equipment required to perform duties of position.
