Schedule:

Monday - Friday, 8:00 AM - 5:00 PM Adecco Healthcare & Life Sciences is currenlty looking for a self-motivated, energetic, detail-oriented, and highlyanized Case Management Coordinator to join our team. This role offers autonomy and flexibility while coordinating care for members. The coordinator is responsible for assessing, planning, implementing, and managing case management activities to support members' overall health and wellness. Position Overview This position involves working with both supportive and medicallyplex cases, requiring strong critical thinking,munication, andanizational skills. Key Responsibilities Conduct telephonic and/or face-to-face member assessments Develop and implement individualized care plans Manage a caseload of members with varying levels of medical needs Coordinate care with Primary Care Providers (PCPs) and specialists Facilitate prior authorizations and referrals Provide condition management education and support Perform medication reviews and ensure adnce Connect members withmunity resources and support services Monitor progress and adjust care plans to improve oues Qualifications Highlyanized with strong attention to detail Tech-savvy with the ability to navigate multiple systems Strongmunication and interpersonal skills Ability to manage multiple priorities and work independently Experience in case management, healthcare coordination, or related field preferred

Pay Details:
