- Works with an interdisciplinary team to achieve treatment goals.
- Utilizes a variety of factors to evaluate patient's biopsychosocial needs.
- Develops a treatment plan and a comprehensive termination of treatment/discharge plan.
- Documents objective and accurate data regarding assessments and interventions.
Contract Details:
13 weeks Full-Time; Monday to Friday 8:30am-5pm.- Must have hospice experience
- at least 1 year.
- Local contract, no travelers accepted for the role
