Full Job Description Job Summary:
The Cardiac Rehab/Wellness Cardiac Tech plays a vital role in delivering comprehensive, patient-centered care by serving as a key liaison between patients, families, and the multidisciplinary healthcare team. This position carries both responsibility and accountability fostering effective communication, and ensuring collaboration to address the physical, emotional, and spiritual needs of adult patients within our practice.
Duties/Responsibilities:
Provide high-quality support during cardiac rehab sessions, including assisting with check-in, collecting vital signs, obtaining medical histories, and monitoring patients as needed. Ensure smooth patient flow by managing check-in/check-out processes and supporting medical staff during patient interactions. Participate in daily team huddles, accepting patient care and administrative tasks including ITP due dates, follow up for signatures, lab follow-ups, and referrals. Handle a high volume of patient communications with professionalism and accuracy, including messages, calls, and escalated concerns. Proactively manage all incoming referrals, lab results, and medical clearances, ensuring documentation is complete and routed appropriately. Assist with daily rosters and class schedules and maintain up-to-date patient records and discharge communications. Actively participate in continuous quality improvement initiatives and contribute to the enhancement of patient care delivery and experience. Routing ITPs and post-discharge communications for required CMS signatures with follow up. Assist with Scheduling rehab and wellness visits, outpatient diagnostic testing, and register patients on the cardiac rehab portal. Discover training needs, provide coaching, listen to feedback, resolve issues, and precept new staff or wellness assistant students. Maintain a safe and clean working environment, ensure adequate supplies, and comply with procedures, rules, and regulations.

Education and Experience:
High school diploma or equivalent (GED).
Preferred:
Credentialed Medical or Nursing Assistant or equivalent education and experience. BLS certification from the American Heart Association. Valid driver's license. Background check required (includes credit, justice, worker's comp, and drug test).

Epic (Cadence, Cupid, Radient, Resolute) experience Computer proficiency in Microsoft Word, Excel, and Outlook.
Physical Requirements:
Ability to assist with adult patients in emergencies as needed. Lifting requirements do not exceed 25 pounds. Prolonged periods of sitting at a desk and working on a computer.
