Job Description

Position Overview:
We are seeking a dedicated and compassionate Children's Low Intensity Care Coordinator to join our team serving Butler and Crenshaw Counties. This position operates within a Certified Community Behavioral Health Clinic (CCBHC) model, focusing on person-centered care coordination, family engagement, early intervention, and connecting children and families to behavioral health and community-based services. The ideal candidate will be flexible, professional, and collaborative, with a passion for supporting children and families in achieving stability, wellness, and access to appropriate care.
Qualifications:
Bachelor's Degree required Strong communication, documentation, and organizational skills Ability to work effectively with children, families, schools, and community partners Ability to work independently while collaborating as part of a multidisciplinary team Knowledge of child behavioral health services, family systems, and community resources preferred Professional, flexible, and team-oriented approach to service delivery
Responsibilities:
Schedule and conduct home visits with children and families to assess needs and support service engagement Gather relevant clinical, social, and environmental information to support individualized care planning Develop and maintain person-centered plans of care focused on the child's strengths, needs, and treatment goals Establish and maintain collaborative relationships with schools, healthcare providers, social service agencies, and community partners Assist children and families in accessing behavioral health treatment, community resources, educational supports, and other entitled services Support continuity of care and coordination between providers to ensure comprehensive, family-centered service delivery within the CCBHC care model Advocate for children and families while helping reduce barriers to treatment and community support services Maintain accurate, timely, and confidential documentation in compliance with agency standards and CCBHC requirements ALL candidates must be able to type and have basic computer skills, possess a valid driver's license, eligibility for coverage under center liability insurance and individually held vehicle liability insurance with a clear background check.
